Dental Plaque Turns to Calculus

In time dental plaque is bathed in saliva which is rich in calcium. The plaque becomes mineralized and forms tartar or calculus. The calculus is very adherent to the tooth and in most cases can only be removed by a hygienist or dentist. The plaque and calculus have abundant bacteria in their matrix and it is this bacteria that causes dental disease including cavities and gum disease. Removing it becomes a priority!

This was removed by our Highlands Ranch hygienist with a dental cavitron (an ultrasonic dental scaler). This was followed by by dental scalers to remove and smooth the tooth. The entire mouth was treated with fluoride varnish to "reduce" the risk of cavities developing.

How does tooth sensitivity start?

Tooth sensitivity starts in early adulthood usually because of gum recession and the loss of cementum covering the dentin of the root surface of the tooth. Dentin tubules (little channels) provide a path to the pulp where the nerve of the tooth is located. Cold or other stimuli travel down these tubules and stimulate the nerve of the tooth causing tooth sensitivity.

How is tooth sensitivity treated?

Dentin sensitivity is treated by applying substances such as potassium ions which depolarizes the nerve. If the nerve is depolarized the need does not react to the stimuli. Another method is by applying materials which seal the outside of the dentin tubules and block off the channels to the nerve. Another method is applying solutions of calcium and phosphate salts to the teeth to reduce sensitivity.

For the most part over-the-counter toothpastes solve the problem of sensitive teeth. Having not seen the patient yet several things came to mind. It is very unfortunate that this patient has let their oral health deteriorate to the point of needing dentures. When we do have the opportunity to evaluate them... dentures may indeed be the only choice. There are some issues that I would like everyone who is in this situation or approaching this level of destruction to know in the following article. A lower denture never fits

For many years our Highlands Ranch dentists have discussed with their patients that a lower denture never fits. The ridge is horseshoe shaped... thin... and resorbs 3X faster than the upper ridge. In addition the tongue constantly dislodges it. Our Highlands Ranch dentists feel that an upper denture for the most part fits O.K. ( didn't say great) but for most patients acceptable.

The bone on the lower melts away

Since the bone on the mandible resorbs 3X faster than the upper what there is to saddle to melts away in time. This is why it is a crime for a young patient to have their teeth extracted at an early age. In fact, over time, the bone can be so thin that the mandible fractures under load. Keeping teeth or dental implants can help preserved the bone.

The "common cold" in dentistry

Dentin sensitivity has been described my many as the common cold of dentistry and by other as toothbrush disease. Still others believe that clenching and grinding of your teeth contributes to this sensitivity. Is has been described as sharp pain arising from some stimulus such as cold or hot... tactile, osmotic (high sugar content e.g.) or chemical (citric acid in orange juice e.g.). It is a common oral problem and affect almost 60% of patients with peaks around 20 to 40 years old. Most of the time it is associated with the facial (cheek side) of the tooth just above the gum line.

Treatment options for teeth sensitivity

Treatment of teeth sensitivity revolves around treating both the cause and symptom. If the tooth sensitivity is due to clenching and grinding you are wasting your effort in placing desensitizers on the tooth until your treat the underlying cause of the sensitivity. Likewise over aggressive brushing with a hard bristle brush needs to stop before any treatment is to be effective.

Types of medicines or medicament's include products for tubule occlusion, protein precipitation and lower nerves sensitivity. The products in treating this problem range from oxalate, calcium phosphate desentizers, sodium fluoride, glutaraldehyde, stannous fluoride and potassium containing products. The above list is boring but it should be noted that there are "various" ways to treat dentist sensitivity.

There have been many innovations in dentistry especially in the last decade. Years ago about our only option was a gold crown. Over time this morphed into porcelain fused to metal dental crowns (PFM). Today, there are a number of very strong non-metal substructures that can be used to fabricate a dental crown. Dr. James DeLapp and Dr. H. Candace DeLapp, Highlands Ranch Dentists, provide non-metal alternatives to traditional porcelain fused to metal crowns. This Highlands Ranch dental office also serves dental patients from Lone Tree, Castle Pines, Centennial, Douglas County, Parker, Ken Caryl and the Denver area.

One of the materials our Highlands Ranch dentist use is “Lava Zirconia” from 3M ESPE. This is a digitally produced zirconia restoration that provides strength and natural aesthetics. The research and application of this material is over 25 years even though this material has only been recently introduced into the dental market. The shading system is available in 8 shades and improves the optical characteristics to make this material look better than porcelain fused to metal crowns.

Another material used in dentistry and can be used for dental implants, as well as natural teeth is Procera. Our Highlands Ranch dentists use this material made from Nobel Biocare. Designing your smile… challenges to color

Dentistry is now becoming both an art as well as a science. In the 1980’s and 90’s form and function were the predominant features that both the dentist and the patient were most concerned with. In addition, dental technologies were not as sophisticated as they are today. Dental porcelains did not have the life-like character as they do today and patients really had no choice.

One of the most difficult things to do in dentistry is to match a dental crown to an adjacent tooth in the front of the mouth especially the two central incisors. In fact it is so difficult that many dentist recommend that the patient consider a veneer on the tooth not treated in order to get some type of color match.

Color of the tooth is but one of the many aspects in matching teeth. In fact it “may” be one of the least important. In addition, most teeth have several colors associated with the tooth. The area up by the gum tissue is … in general… more yellowish. This is due primarily to the underlying yellow dentin showing thru the translucent enamel on the tooth. Like wise… the incisal edge of the tooth can appear grayer because the translucent enamel has no backing of yellow dentin and the darkness of the mouth give the edge a gray color.

The following is an example of how we use multiple colors on a tooth as well as irregularities in the surface texture of the tooth and differing translucencies of the tooth. This case highlights the difficulties encountered in designing your smile.

How we handle this is to send to the dental laboratory two items. The first is the digital photograph of the tooth showing the:

Overall color of the tooth
Colors in different parts of the tooth

The value of the tooth (relative brightness of the tooth)

Opaqueness of the tooth

Translucency of the tooth
White “lines’ in the tooth

White “lines” on the edge of the tooth
Any “other” colors or unique characteristics

The second is to draw attention to the factors described above… we also include a “color map of the tooth. A color map is a drawing of the tooth with the unique characteristics that we found on the tooth that we wish to emphasize to the dental laboratory.

Armed with this information, the dental laboratory will incorporate this in their artistic product. However, understand that this is one of the most difficult endeavors in dentistry. We discuss this with patients and at times the best solution ”may” be also doing a veneer on the other central incisor.

Technology has changed tremendously in the last twenty years. The dental intra-oral camera is just one of these innovations. The intra oral camera allows the dentist and the patient to get a close-up look at areas of special interest in the patient’s mouth. As a dental friend once remarked… “to see is to know… to not see is to guess”. When we can “see” the worn out filling on a television monitor it becomes apparent what needs to be done. The intra-oral dental camera is designed to go inside your mouth and it is approximately the size of the dental handpiece or dental mirror. In addition, it has a light built in to the camera which illuminates the area to be examined. This camera has the ability to take a video or a series of still photographs of the area.

Most cameras have the ability to zoom in on one tooth or… give you a video tour of your entire mouth. In our dental office your images are displayed on a television monitor that is mounted directly above the dental chair. The images seen include but are not limited to:

Broken fillings
Broken teeth
Worn fillings
New decay (cavities)
Tartar (calculus)
Plaque
Dental lesions of various types
How the teeth fit together
and ... much more!

In the digital age we are able to print out a series of photographs for you if you wish. On occasion these photographs may be sent to insurance companies or to a referring specialist’s office.

The bottom line is extra and intra-oral imaging is… “you can see what we see”. There is nothing better than having both the doctor and the patient on the same page and understand what needs to be done.
